Health Requirements

In accordance with Illinois State Law, students entering kindergarten, sixth grade or new to the district must have a physical examination (recorded on the Certificate of Child Health Examination form). Immunizations must be complete and up-to-date according to the schedule of the Illinois Department of Public Health. The health history section of the form must be completed, dated, and signed by the parent/guardian. The Certificate of Health Examination form is due PRIOR to the first day of school. Families will be informed of the State of Illinois School Exclusion policy that outlines details of student’s exclusion from school until the above health information is completed and filed with the school nurse. Students transferring from out of state or another country must present a physical exam completed within one year prior to Illinois school entrance on a comparable form and must comply with the immunizations schedule. All transfer students shall have 30 days following registration to comply with the health examination and immunizations requirements. All children entering kindergarten, second, and sixth grade are required to have a dental examination. A vision examination is required for all students entering kindergarten or a public school for the first time. All examinations must be reported on the appropriate forms, which can be downloaded from the district website (www.lf67.org) or picked up from the district office during the summer.

If your child requires any type of medication during the school day, a Medication Authorization Form must be completed by the parent and signed by the prescribing physician. No medication (prescription or non-prescription) can be given to your child at school without these forms on file. Medications forms are available on the District web site.

Physical exams are required each year for students who are participating in any after-school sport.  Sports physical exams are good for only one year from the date of the exam.  Exam forms must be on file in the school nurses’ office before the student will be allowed to participate. Please make appointments early so your child’s health record is complete by the start of school. It is advisable to make copies of forms for your own records.
